Mobile appointments are offered in Bedfordshire and Essex.
Please contact me if you are requiring a booking outside of the areas mentioned above as I do travel to treat outside of these main areas on occasion.
Veterinary Consent Form
Prior to me seeing your animal (horse/dog) I require a consent form for treatment to be signed by your dog's veterinarian and returned to me with a copy of your dogs clinical history. Please click below to download the form. Once downloaded, fill in all the information you can and email/hand deliver the form to your veterinarian for completion. After the form is filled, it need to be returned to driscolltherapy@hotmail.com
